Patient Consent Policy
By ticking the consent checkbox on the booking form, you confirm that you have read and agree to the following. The date and time you tick the box is recorded with your appointment as proof of consent.
1. Consent to tele-consultation
You consent to receiving medical advice through video or phone consultation. You understand that tele-consultation has limitations — there is no physical examination, and the doctor relies on the information and documents you provide.
2. Use of personal and health information
- You consent to your personal details, medical history, vitals, lab reports, and consultation notes being collected and used solely to provide your care.
- Your information is stored securely and is accessible only to the consulting doctor and authorised clinic staff.
- We do not sell or share your information with third parties for marketing purposes.
3. Identity verification
You consent to providing your Aadhaar number for identity verification and for linking your records across visits. Aadhaar is stored securely and is never displayed in full to other patients.
4. Communication
You consent to receiving appointment-related communication (confirmations, prescriptions, follow-ups) on the phone number and email you provide.
5. Not for emergencies
You understand that tele-consultation is not for emergencies. If you may have a medical emergency, call 108 or visit the nearest hospital immediately.
6. Withdrawing consent
You may withdraw consent at any time by contacting the clinic. Withdrawal will not affect care already provided based on prior consent. Records will be retained as required by applicable law.
