- Recent lab reports as PDF/JPG/PNG (FBS, PPBS, HbA1c & Serum creatinine are highly recommended if your consultation is for diabetes or prediabetes)
- Your weight, height, and recently measured blood pressure.
- Brief reason for consultation / main concern
- A working email to receive notifications, prescription, etc., and a WhatsApp-capable phone number for tele-consultation.
- A few quiet minutes — the form takes about 3–5 minutes
